Family Case Management

Family case management is a statewide program funded by the Illinois Department of Public Health and the Illinois Department of Public Aid.  Family case management provides help to pregnant women and children up to age 2.  Services provided through the program include:

·        Help obtaining medical care.

·        Follow-up with medical referrals.

·        Teaching related to prenatal needs, baby care, and parenting.

·        Baby assessments (weight, measurement, developmental screening).

·        Referral to WIC, Public Aid, educational programs, and other needed community services.
